By design: the chief complaint and the diagnosis are carried across word for word from what was said. The model summarises; it never invents a finding or a diagnosis.
Indian clinics do not consult in one language, so the scribe does not listen in one. The Premium engine is tuned on Indian accents and on sentences that switch language halfway through.
